Appointments:
- Book your appointment in advance in order to reserve your requested date and time.
Please read the following carefully:

Client Arrivals :
- In order to maximize your time in our office, we ask that you fill out your forms at home prior to your appointment.
- Plan to arrive at least 20 minutes prior to your appointment in order to park, relax and enjoy our amenities.
- Forms can be downloaded and printed from the Download Patient Form link above. If preferred, we can mail the forms to you.

Late Appointments:
- If you arrive more than 15 minutes late for an appointment, you may be need to be re-scheduled.

Rescheduling, Cancellations and "No Shows":
- Rescheduling - All pre-scheduled surgical and cosmetic consultations and procedures must be rescheduled with our receptionist at least 24 hours in advance.
- Cancellations and "No Shows" - If a cosmetic consultation or procedure is cancelled with less than a 48 hour notice, or if you are a "no show" for a scheduled cosmetic consultation or procedure, the credit card on file will be charged.
Fees for "no show" or cancellations with less than 48 hours notice for appointments with non-MD providers (Physician Assistants, Patient Co-ordinators, Estheticians) will be $125.
Fees for "no show" or cancellations with less than 48 hours notice for appointments with MD providers will be $185.
